A Narrative Bedside Examination for Vestibular Neuritis

Abstract
|

|
Vestibular neuritis is characterized by disabling, sustained vertigo and imbalance with an acute or subacute onset.1
It is the second most common etiology of peripheral vestibular disorder with an annual incidence approximating 3.5 cases per 100,000 persons.2 Since the clinical features of a central lesion can occasionally simulate vestibular neuritis (i.e., vestibular pseudo-neuritis), it requires immediate triage during the acute phase. Even though neuro-ophthalmology trainees are often encountered by patients with vestibular neuritis, the diagnosis has been mostly reversed for neurotology specialists. Fortunately, an essential bedside neurotologic examination can easily guide the diagnosis in most cases, although it remains a huddle for the trainees. Herein, we introduce a patient with vestibular neuritis and explain the clinical approach in detail focusing on the practical aspects. We first address the patient history and basic neurotologic examination, then discuss the management and prognosis.
